In order to save time during your initial session, please complete the Intake Form before you arrive for your appointment. This is required for ALL clients. The Consent to Treat a Minor Form must be filled out if anyone under the age of 18 is participating in therapy. A legal guardian must give consent for the treatment of the minor. All adolescents and teens participating in group therapy must have a legal guardian complete the form before the start of the first group meeting.
